PLEASE NOTE: You ARE NOT required to sign or send in the Consent for Disclosure form to get Free/Reduced Child Nutrition Program benefits for your children. If you do not sign the Consent for Disclosure, it WILL NOT affect eligibility for or participation in the Child Nutrition Programs. HOWEVER, to save you time and effort of filling out more paperwork, information about your children’s eligibility, that has already been decided for Free/Reduced Child Nutrition Program benefits, may be shared with other programs for which your children may qualify, but only with your consent. For Free/Reduced Instructional Materials, please complete this Consent for Disclosure to give us permission to share your information.
